While scheduling your new patient appointment, one of our friendly receptionists will offer to email you our new patient paperwork to be completed at your convenience. This process helps us prepare for your visit and ensure a smooth and efficient first visit.
If You Prefer to Fill Out Forms In-Office
If you are unable to complete the forms online before your visit, please arrive 10-15 minutes early to your appointment. This extra time will allow you to fill out the necessary paperwork in our office without delaying your scheduled time with Dr. Hansen.
What to Bring to Your First Appointment
To ensure we have all the information needed for your initial visit, please bring the following items:
- Completed New Patient Forms: If not submitted online.
- Insurance Card and Photo ID: To verify your coverage and identity.
- Recent X-Rays: If available, especially if taken within the last year.
- List of Medications: Any medications you are currently taking.
- Parent or Guardian: Required for patients under the age of 18.

During your appointment, Dr. Hansen will conduct a comprehensive oral evaluation, including an oral cancer screening, dental and periodontal analysis, and a head and neck exam. We also provide personalized oral hygiene instructions and a warm towel after your appointment to help you relax and refresh.
